Title: Anxiety Reduction in Dialysis Induction Continuous Quality Improvement Project: Pilot Study

Abstract

Individuals with End-Stage Renal Disease (ESRD) undergoing renal replacement therapy often experience heightened anxiety levels during dialysis initiation. This pilot study outlines a Continuous Quality Improvement (CQI) project aimed at reducing anxiety and enhancing the overall experience for these patients. 

Objective: To implement and evaluate a psychoeducational program designed to alleviate anxiety in individuals undergoing dialysis induction. 

Methodology: The project follows a CQI framework and comprises three key components. Firstly, an integrative literature review using the JBI method informs the foundational knowledge. Secondly, the translation and cultural adaptation of the Kidney Disease Knowledge Survey (KiKs) into European Portuguese facilitate culturally sensitive assessment. Lastly, a pilot study involving 65 inpatients undergoing hemodialysis induction at a University Hospital's Nephrology and Renal Transplantation Service evaluates the effectiveness of the psychoeducational program. Instruments include the Mini-Mental State Examination, Problem-Solving Inventory, Frailty Scale, KiKs, Hospital Anxiety and Depression Scale (HADS), and informal interviews. Ethical approval and informed consent were obtained before initiating the study.

Results: Pre-intervention, anxiety scores averaged 6.52, reducing significantly to 2.32 post-psychoeducational sessions. Knowledge deficit regarding self-care and symptom management decreased from 27% to 5% after counseling sessions. The pilot study demonstrates promising results in anxiety reduction and knowledge enhancement. 

Conclusion: This CQI project lays the groundwork for a comprehensive intervention to address anxiety during dialysis induction. The psychoeducational program exhibits potential benefits, providing insights into effective strategies for improving the overall well-being of individuals entering dialysis. Further research and refinement of the program can contribute to advancing the quality of care for this specific patient population.
