Title: The Effect Of Changes In Co2, Plasma Protein, And Hemoglobin Towards Increasing Blood Acidity In Respiratoric Acidosic Patients Taking Attention In Icu Room Dr.Slamet Martodirjo Pamekasan,Indonesia

Abstract

At this time, the incidence of respiratory acidosis has a fairly high mortality, which is around 40-60%. Death in patients with respiratory acidosis is influenced by various predictable factors. One of the factors is an increase in blood acidity for a long time so that the patient experiences a decrease in consciousness. In addition to the increased factor of CO2 nonvolatile factors such as protein and HB levels can also affect the level of blood acidity, this can occur in patients with respiratory acidosis in hospitals who experience other clinical conditions that aggravating the patient's condition, such as anemia, hypoalbuminemia which easily occurs due to malnutrition due to chronic disease.  A preliminary study conducted by researchers at the RSUD dr.H.Slamet Martodirdjo Pamekasan was found in 2019-2020, the number of respiratory acidosis patients treated in the ICU room was 266 people, who died in 34% of patients.

The method used in this research is observation analytic research. In this study, the population was all acidosis patients who were treated in the ICU room at RSUD Dr. Slamet martodirjo Pamekasan as many as 266 people.In this research,there are 30 samples,  And using a non-probability purposive sampling technique, data collection in this study was through observation of medical record data. The diagnosis of respiratory acidosis was made according to the Henderson-Hasselbalch criteria, namely PH < 7.35 and Pa CO2 > 40 mmHg. Inclusion criteria were patients aged ≥10 years old, meets the criteria for diagnosis of Acidosis   Respiratory acidosis or respiratory acidosis with complaints of disease in the respiratory organ system, receiving treatment in the ICU, and having medical record data, Inclusion criteria were patients aged 10 years, met the diagnostic criteria for respiratory acidosis or acidosis with complaints of disease in the respiratory organ system, received treatment in the ICU, and had medical record data (blood gas lab, Hb, albumen).

Although partially plasma protein and Hb have no effect, the results of the statistical linear regression test simultaneously reveal the opposite fact that by using the F test it can be proven that the PaCO2 , plasma protein and Hb variables together have a significant effect on increasing acidity with a level of significance. P=0.041.

 It is hoped from this study that nutritional management in acidosis patients should be considered intensively to reduce the negative effect of malnutrition on blood acidity so that the albumen and Hb buffer system can maintain normal blood acidity.

Taufiqur Rahman has completed his Magister of health at the age of 37 years from STIE Malang University, Indonesia. He is the head of medical surgical research  department  of  Madura Polytechnic, Indonesia. He has over 30 publications that have been cited over 50 times, and his publication h- index Sinta is 125. He is a lecture medical surgical nursing and critical nursing.
